Below are links to Registration Forms for Religious Education classes
 for 2020-2021 school year. 

These are fillable forms, which can be completed and printed, or the blank can be printed and filled out by hand.  If completing by hand, please print clearly.  If you don’t have access to a printer, please call the office to request the form.

Bring the printed and signed forms to the parish office along with the necessary fees.  If the office is not open, please put the form and payment in an envelope and put through the slot on the East door of the office (faces the parking lot.)

For Registered and Active members of San Martin Parish:

For families who are NOT registered or NOT active members of San Martin Parish:
Each child …………………$40
 

Additional Fees:  For those receiving Sacrament of Baptism, First Holy Communion and/or Confirmation there will be a $10 fee per certificate that must be paid before
March 30, 2021.
